Center for Health Impact and EMK Community Health Center Release CHW Toolkit
CHW employers need a standardized way to orient new hires. The CHW Orientation Toolkit was created to meet this need.
Development of the Toolkit was funded by a Health Care Workforce Transformation Fund grant through the Commonwealth of Massachusetts, Executive Office of Labor and Workforce Development. The grant program was administered by Commonwealth Corporation. The Center for Health Impact was the lead on the grant and Edward M. Kennedy Community Health Center was the employment partner.
The Toolkit includes detailed CHW job descriptions as well as an orientation timeline and basic education guidelines; CHW-specific policies and procedures; standards for CHW performance of assessment, documentation and data collection; guidance for how to communicate with patients, coworkers and partner agencies; resources and links to additional information.
The Toolkit is intended for use by other agencies as a template to assist with developing agency-specific CHW orientation activities.
